In mid-2019, a National Health Plan and Everly Health Solutions partnered to provide at-home collection kits to eligible Medicare Advantage members to increase preventative screening rates. Test offerings included screenings for colorectal cancer, the management of blood sugar levels, as well as kidney function testing in diabetics.

The Challenge

In mid-2019, a National Health Plan and Everly Health Solutions partnered to provide at-home collection kits to eligible Medicare Advantage members to increase preventative screening rates. Test offerings included screenings for colorectal cancer, the management of blood sugar levels, as well as kidney function testing in diabetics.

The program’s goal was to drive higher testing completion rates for Medicare Advantage Members, create an accessible end-to-end testing and results experience, and improve customer satisfaction rates.

This equips members to identify and potentially prevent early complications related to diabetes and colon cancer, reducing the burden of future healthcare costs and improving health outcomes. While at the same time, the National Health Plan could improve or maintain its Medicare Advantage star rating score to 4 or higher, which influences future enrollment of new members, as well as grants access to a 5% boost to monthly per-member payments from Medicare.

The Solution

Everly Health Solutions conducted omnichannel member outreach that included phone calls, text, and email communications as well as providing educational resources to encourage test opt-in and completion to drive increased routine preventive screening.

Members who opted in were sent an Everly Health Solutions at-home collection kit within 2 days. Each test kit comes with easy-to-follow instructions for a simple sample collection experience, digital results delivered through our intuitive HIPAA compliant results platform (results were also available by mail), and meaningful insights on possible next steps.

The Everly Health Solutions results platform provides members with educational resources that break down exactly what biomarkers were measured, what the test results may mean, how they may affect their health, and what next steps they may be able to take—so members can immediately begin to take action on their health.

Everly Health Solutions also supported the National Health Plan with member insights and data to better understand what led members to complete tests and consolidated the National Health Plan’s test kits and member engagement vendors into a one-stop-shop, relieving operational burdens and resources. The National Health Plan closed gaps, and members prioritized their health. A win-win.
